Consent Form
 
If your child would like to attend any of the Machan Trust groups they must have an up to date consent form completed. Once this form has been handed to a member off staff it allows young people to access any of the groups that we run for their age group. An electronic copy of the form can be downloaded to the right and returned by email.  Or a form can be downloaded for printing and writing and handed in to the Machan Trust.  Alternatively a form can be collected from the Trust and completed in writing.
